BIRTH AND Caesarean Section
NORMAL BIRTH
I call NORMAL BIRTH any type of birth in which the mother and the baby are reunited smoothly and happily, the family and the birth team smile, desserts are eaten, and our photos of joy are shared with pride. Whether born vaginally or abdominally. Is vaginal birth normal or cesarean birth abnormal?
I feel bad for the vaginal births I had during my two pregnancies. Both were abnormal. In one case, I insisted on a vaginal birth and the baby turned out to be very unwell and remained in the incubator. In the other case, my patient insisted that my vagina was torn to pieces. I repaired the cervix, vagina and vulva for hours while I was asleep. My patient could not walk for 1 month. He finally had surgery. Not every pregnant woman has a normal birth just because the baby comes out of the vulva. It is necessary to look carefully at the result.
There are very few women who can give birth vaginally without the intervention of a physician or midwife.
Our living conditions have changed a lot in the last 10 years. We don't get sunlight, we don't walk, we don't work in the field or garden, our legs get tired even in the Turkish toilet and we have even forgotten the knee-elbow position. We may be surrounded by women who constantly spread fear, negative emotions and information.
RECEIVING PREGNANCY, BIRTH, POSTPARTUM EDUCATION
It is very beneficial for women who want to give birth vaginally to have a great mood before birth and to be conscious about their desires, exercises and nutrition.
With my 25 years of experience as a gynecologist and 1.5 years as a child specialist, I cannot give up on either the mother or the baby. I attach great importance to the fact that mothers and fathers receive pregnancy and birth education, whether it is a vaginal birth or a cesarean birth.
We carry out these trainings very well together with our midwives within Girişgin Family Consultancy. During the epidemic, we can accommodate individual families or 3 families in a large hall, strictly adhering to distance and antiseptic rules.
Risky pregnancies are determined by me before birth, and preparations for cesarean section are made in advance.
VBAC (Vaginal Birth After Caesarean Section)
If our older pregnant women are considering VBAC, of course, it is necessary to inform them about it and to evaluate and be determined beforehand if they are suitable for vaginal birth. While VBAC was never talked about in Turkey, between 1997 and 2000, we had a very high number of VBAC mothers that we published with our teacher Ali Acar, who is a professor today. With my private practice experience, I have discovered that some of our pregnant women give up halfway through. Their mothers and husbands can give up. They cannot maintain the determination to endure the birthing process.
Since VBAC monitoring is difficult and risky, of course, the fee is high. You have to sign extended consent forms, which are pretty risky. By planning VBAC and giving 5-6 units of blood to pregnant women whose uterus had ruptured and they were almost dying, their lives could only be saved. Babies also died.
But the families never blamed the team. Because they chose it.
As a gynecologist, I am a physician with enough experience not to make decisions that could endanger the mother or the baby, or both. I spent my life on duty in state maternity hospitals for years. VBAC cases in which the uterus ruptures, the baby dies, and the woman loses her uterus are never shared in the media.
Epidural and spinal anesthesia have greatly increased the luxury of cesarean section. Now, just like a vaginal birth, staying in the hospital for 1 or 2 days is sufficient.
Skin-to-skin contact, seeing the baby and hearing his voice are very easy. I don't believe there is any difference in breastfeeding.
Breastfeeding requires a very special awareness, determination and sacrifice. I have seen many women who gave birth vaginally and did not breastfeed their babies for more than 2-3 months, and I also met mothers who had a cesarean section and even worked and breastfed their babies wonderfully for 2.5 years.
